Removing barriers to care and helping people live healthy

Abbott’s Future Well™ Communities initiative is finding real-life answers to chronic disease by addressing the everyday challenges that prevent people from living healthy.

With support from Abbott Fund, local efforts began in Stockton, California, where 60% of people are living with diabetes or prediabetes. An important part of this work is Abbott’s Healthy Food Rx program, a “Food Is Medicine” initiative delivering healthy food boxes along with nutrition education.
