EMPLOYMENT APPLICATION - ABBOTT LABORATORIES & SUBSIDIARIES

AS USED HEREIN, "ABBOTT" SHALL REFER TO ABBOTT LABORATORIES, AN ILLINOIS CORPORATION AND ALL OF ITS SUBSIDIARIES.

I hereby certify that the electronic signature below has been adopted by me and is the legal binding equivalent of my handwritten signature. I acknowledge that I am accountable and responsible for actions initiated by me under my electronic signature.


This acknowledgment also applies to handwritten signatures that I may execute to electronic records. I agree to follow all Abbott's security requirements for electronic records.


I will not share nor reveal my passwords or personal identifying information to others, and if I suspect that my password, account, or personal identifying information have been compromised, I will promptly notify Talent Acquisition.


I agree to utilize this electronic signature for all transactions required by Abbott. I understand, however, that I will have the option to print this Verification and Acknowledgement once I have entered my electronic signature below, and that I can request a paper copy from Talent Acquisition.


I understand that I have the right to withdraw my consent to proceed with an electronic signature at any time without any consequences by contacting Talent Acquisition.

NOTICE REGARDING JOB APPLICANT AND EMPLOYEE DRUG SCREENING

Abbott manufactures controlled drugs and health care products. As a matter of policy and to help ensure a work environment free of the unlawful use of controlled drugs, Abbott screens job applicants for the presence of certain drugs.


All job applicants are required to successfully complete a drug screen before employment with Abbott. Applicants refusing to consent to pre-employment screening for the presence of controlled drugs will not receive further consideration for employment at Abbott. This policy does not apply to the use of controlled drugs within the limits of a valid prescription.


Positive test results, confirmed by an independent laboratory retest, and not resolved to Abbott satisfaction by an adequate explanation such as a valid prescription, will result in termination of candidacy for employment at Abbott. Such applicants will not be reconsidered for a period of one year.


Employee involvement in a serious accident and behavior or disregard of known rules which could result in a serious accident will be considered cause for drug testing. Any indication of unlawful drug use or abuse of prescription drugs will be considered cause for drug testing. Unexplained deterioration of performance and return to work after any leave or suspension will also be considered cause for drug testing. Group testing may also be required where incidents described above occur involving one or more members of the group or in other situations determined by management to be indicative of unlawful use of controlled drugs among any or all members of the group. Abbott reserves the right to test employees in certain job categories where liability risks or safety of others are involved, or where required by statute or regulation. Abbott also reserves the right to amend its drug policy at any time without notice and to require broader drug testing of its employees in the future including, but not limited to, random and across-the-board testing.

I hereby authorize all public and private individuals, courts and law enforcement agencies, corporations, organizations, firms, institutions and agencies who possess information about me, to release to Abbott, or any designated representative thereof, without liability, any and all information concerning the aforementioned areas of inquiry, and to allow copies to be made of any requested written data.


I also understand that any information furnished in this application or obtained as a result of any inquiry will not necessarily preclude my employment, but will be used as part of an overall evaluation of my qualifications.


I certify that I have not knowingly withheld any information that might adversely affect my chances for employment and that the facts contained in this application are true and correct to the best of my knowledge. I understand that, if I am offered a position or employed, false, concealed, or omitted statements on this application shall be grounds for rescission of offer or dismissal, no matter when discovered, and that false statements on this application may preclude my employment at Abbott.
